Agenda Item #57 Item Title C-65 Change Order #1 for Doctors Inlet Elementary Parking Improvements (Revised) Issue/Rationale Change Orders are initiated by the Contractor, Architect/Engineer or Owner, and may increase or decrease the scope of the project as defined by the plans and specifications. Once a Change Order is initiated, review/evaluation is undertaken to determine the merit and/or requirement of the Change Order. Change Orders are reviewed by the Architect/Engineer and staff prior to submission to the School Board.
Change Orders affect the scope of the project and are required to be School Board approved by State Requirements for Educational Facilities and Florida Statutes. This Change Order is for water main revisions, relocation of water bibs/shrubbery, delays due to conflicts with school testing, rerouting of storm drainage and a mutually agreed to delay to minimize affect on school when phasing traffic plan or busses and parent pick up areas. Contractor: Jacksonville Eighteen Construction, Inc.; Engineer: Mittaur & Associates, Inc. Recommended Action Approve the Change Order. Financial Impact If approved, this Change Order will increase the contract amount by $12,674.35. The Substantial Completion date is June 30, 2006.
