C-9 Renewal of 2015-2016 List of Physicians and Medical Facilities authorized to conduct physical examinations and drug tests required for certain groups of employees
Issue/Rationale
School Board Policy and Florida Statutes require physical examinations and drug tests for certain employees and job candidates. Physicians/Facilities used in administering physical examinations and drug testing of employees and fees charged for those services must be approved by the School Board. Twenty-one (21) requests to submit a bid to conduct district physical exams were sent to local physicians in Clay County and/or surrounding counties. The charge shown on the attachment are the physicians and quotes received. They are considered reasonable and are recommended for approval.
Recommended Action
Approve the Physicians/Facilities and costs presented.
Financial Impact
Approximately $64,290 has been budgeted for physical examinations and drug/alcohol tests for certain employees and job candidates. The quotes received and recommended are considered reasonable.
