C12 - Keystone Behavioral Pediatrics Amendment for Extended School Year 2017.
Description

The IEP team has provided a series of INCREASINGLY INTENSIVE supports for this youngster.  The necessary services for this student include EXTENSIVE behavioral supports.  This Amendment with Keystone Behavioral Pediatrics will serve one Clay County student in this alternate educational placement.  Providing highly specialized supports required by the student’s Individualized Education Plan (IEP), this amendment covers Extended School Year services (ESY) through July 27, 2017.

Gap Analysis

The IEP team has provided a series of increasingly extensive supports for this youngster. The necessary services for this student include intensive behavioral supports provided by a team of highly trained professionals in an intensive therapeutic environment.

 

Previous Outcomes

A diverse number of placements have previously been utilized for this student.  The IEP team has determined greater supports are necessary in order to provide a free appropriate public education (FAPE).  

Expected Outcomes

Placement at Keystone Behavioral Pediatrics for 2017 Extended School Year (ESY) is expected to meet the student’s IEP goals and provide FAPE.
