The School Board of Clay County, Florida is in need of health care services on-site at Wilkinson Junior High School. AZA Health is willing to provide primary and preventative healthcare services both on-site and also through telehealth, for students enrolled at Wilkinson Jr. High (parental consent is required) and also for members of the surrounding community. The services will be provided year-round on the school campus with no fewer than 20 hours per week of service at the school campus. The School District has built out a clinic on the campus which will be utilized by AZA Health. AZA Health will be solely responsible for all operating expenses including, but not limited to, AZA Health staff salaries, office supplies, medical supplies, janitorial services, telecommunication services and disposal of medical waste. SBCC shall not pay or become obligated to pay, because of or in connection with this MOU, AZA Health’s operating expenses. The School district will be responsible for providing appropriate facility space (Building 92-43) on the Wilkinson Junior High School campus to accommodate the health care services operated year-round by AZA Health, with an adequate power supply, plumbing, running water and HVAC. AZA Health agrees to pay a nominal yearly fee at a mutually agreed upon rate for rent if requested. The funding is provided by Children’s Home Society through a grant obtained by CHS.
