C18 - Agreement between the 21st Century CCLC Grant Program of the Clay County School District and Health-Tech Consultants, Inc for the 2016-2017 school year.
Health-Tech (third party evaluator) will provide services for data collection, data analysis, and reporting as necessary to complete the evaluation process identified in the 21st Century CCLC Project Evaluation Plan.
