Item Title
C-15 School Affiliation Agreement between CCSD and Orange Park Medical Center

Issue/Rationale
This agreement is to allow the students in the Health Occupations programs in the high schools to do clinicals at the Orange Park Medical Center. The clinicals will provide classroom theory and practical instruction to students, prepare student/patient assignments and rotation plans for each student, continue oral and written communication with hospital regarding student performance and evaluation, and supervise students and their performance at hospital.

Recommended Action
Approve School Affiliation Agreement as presented.
