Addendum initiated by Heather McDonald, previous Supervisor of Mental Health, to update form Exhibit B on the contract agreement between the School Board of Clay County, FL and Flagler Hospital to include a website link for reference of fee rates at the Current Florida Medicaid fee schedule and the Community Behavioral Health Services Fee Schedule on the Florida Agency for Health Administration Medicaid website per Section 5 - Compensation and Payment by Third-Pary Sources.   

Elimination of contract disruptions or non compliance as a previous contracts with set rate fees were required to change immediately upon the change of the fee rates.   This enables more fluid contract process and uninterrupted service provision.
