Item Title
C-31 Change Order #1 for Orange Park Junior High School Fire Alarm Replacement (Revised)

Issue/Rationale

Change Orders are initiated by the Contractor, Architect/Engineer or Owner, which may increase or decrease the scope of the project as defined by the plans and specifications. Once a Change Order is initiated, review/evaluation is undertaken to determine the merit and/or requirements of the Change Order. Change Orders are reviewed by the Architect/Engineer and staff prior to submission to the School Board. Change Orders affect the scope of the project and are required to be School Board approved by State Requirements for Educational Facilities and Florida Statutes.

The client sketch and engineering drawing did not show all the fire alarm devices that needed to be changed. These changes are needed in order to pass the Fire Marshall inspection. Additional information is in the attachment.



Recommended Action
Approve Change Order #1 as submitted.

Financial Impact
$7,170.00.  This addition to the construction contract will be paid from Special Maintenance Section IV funds.
